More detail about the role The hospital discharge team are based at the main Royal Devon university healthcare trust. They operate a seven-day service that provide a single point of contact for patients, families, carers and associated people, the team facilitate and co-ordinate the safe and timely discharge from Hospital for our patients in partnership with other multi-disciplinary colleagues. The team are based within the Community Division which is spread across the Devon area ranging from Okehampton (Mid Devon) across the City of Exeter to the Seaside town of Seaton (East Devon). About the opportunity An exciting opportunity has arisen for a discharge co-ordinator to work 37.5 hours per week within our hospital discharge team. The Discharge Co-ordinator is an essential role within the team and you will form part of a supportive and close working group of professionals working in partnership with other multi-disciplinary colleagues across the Eastern Area. You will be working with the team to increase patient flow and optimise patient experience surrounding transfers of care. As a team of Therapists and Nurses we provide face to face triage and clinical assessment of patients to support their onward care needs. This is an exciting role that has a great deal of variety and scope within the working day. About You We are looking for an enthusiastic, motivated and caring person with excellent communication skills to co-ordinate and support the team, an ability to work in a fast-paced environment and problem-solve with be essential, the role will include documenting actions on meetings, co-ordinating the teams workload and liaising with a variety of health & social care colleagues across the Eastern area. You will be required to answer the telephone and respond to e-mail enquiries in a professional and timely manner. A knowledge and understanding of the health and social care environment would be desirable although not essential.
